Health condition
Older psychiatric patients with severe mental disorder who refuse help.

Outcome measures
Primary outcome
1. Number of patients who receive care and the number of patients who stay in care;
2. The psychiatric-, somatic condition and social functioning of the patient;
3. Quality of life.
Secondary outcome
1. Ability to live and do things independently;
2. Crisis contacts;
3. (Court ordered) hospitalization and hospitalization time;
4. Increase of motivation for treatment and medication compliance;
5. Stronger therapeutic relationship.
Background summary
A randomised controlled trial examines the effect of Assertive Community Treatment for older psychiatric patients with seriously various problems who refuse help compared with care as usual.
Study objective
As compared to care as usual
Assertive Community Treatment for the elderly creates:
1. Dropout prevention;
2. Decrease in psychiatric symptoms;
3. Decrease in somatic symptoms;
4. Decrease in social problems;
5. Increase in Quality of Live.
Study design
Baseline, 9 months after randomisation and 18 months after randomisation.
Intervention
Assertive Community Treatment.

Inclusion criteria
1. 65 years or older;
2. patients with a psychiatric diagnosis (or strong supposed diagnosis) on Axis I of the DSM IV-TR;
3. lack of motivation for treatment;
4. regular treatment is not sufficient;
5. seriously various problems.
Exclusion criteria
1. Moderate to seriously dementia;
2. Simple addiction.
