Intervention
Outcome measures
Primary outcome
1. Loss of functioning (ADL/IADL);
2. (Unnecessary) Hospital complications;
3. Length of stay.

Background summary
Early detection of problems and adequate coaching of frail elderly people is crucial to prevent unnecessary loss of function, decrease in quality of life, and even death in old age. This is especially true for frail elderly people who are admitted to the hospital. These patients typically have complex health problems that require a multi-dimensional approach. By visiting these elderly people with a consultation team which is specialized in geriatric problems we try to prevent unnecessary loss of function decrease in quality of life, and even death in old age.
First we will include the control-group and after the intervention is introduced, the intervention group is included. For both groups an inclusionperiod of 3 months will be used.
Our primary outcome measures are:
1. Loss of functioning;
2. (Unnecessary) hospital complications;
3. Length of stay.
Study objective

Study design
1. Within the first 72 hours of hospitalization;
2. 3 months after hospitalization.
Intervention
Geriatric consultation team will visit the elderly people in the intervention group. The geriatric consultation team identifies problems and possible complications and initiates actions that are aimed to prevent loss of functioning and (unnecessary) hospital complications.

Inclusion criteria
1. Age 70 and older;
2. GFI 4 or higher.
Exclusion criteria
1. No informed consent;
2. Terminal illness.
