Primary outcome
the interrater reliability will be calculated with kappa and will follow the following values <40 bad, 0,41-60 reasonable, 0,61- 0,80 good and 0,81-1,00 very good interrater reliability. For calling sensitivity and specificity high a value of 80% and higher will be used. A positive LR >10 and a negative LR <0.1 will be used as high significance for the likelyhood whether or not the condition is present.

The simple shoulder exam might be a helpfull tool to use at primairy care
Background summary
In 2013 orthopedic surgeon Huijsmans developped a simple shoulder exam. This simple shoulder exam is used at Bergman clinics, but has not been researched on reliability and validity. With this research we want to examen the interrater reliability and validity of this model and get an impression if it can be used by physicaltherapists.
Method: The research will be carried out in the clinic from orthopedic surgeon Huijsmans with 75 cliënts with shoulder pain who come for their first consult. All cliënts will go trough the exam twice, first by the physicaltherapist after that by the orthopedic surgeon as part of the usual care. To make sure the study is blinded the PT will leave the room after she is done with her part of the research. Directly afterwards the orhtopedic surgeon will start the usual care. Cliënts are free to refuse to participate and can stop their participation at all times. All the data is saved in Castor and every test subject is given a personal code so their identity is concealed.
The interrater reliability will be calculated with a kappa coefficient and percentage of agreement. The validity will be calculated by sensitivity, specitivity and likelihood ratio.
The difference with the usual care is that the PT asks two questions and executes a maximum of four physical tests before the consult with the orthopedic surgeon.
The chance on increase of pain is minimal because the PT is has 20 years of experience and will performe the tests meticulousness.
The exams from the PT and orthopedic surgeon are taking place onde after the other. The benefit is that there is no natural course of the disease. Possible disadvantages can be that the test subjects can remember the outcome from the first tests and take that into account during the second examination and there might has been a change in the mobility of the shoulder.

Inclusion criteria
- Clients with shoulder pain
- Clients are older than 18
- Clients who come for their first appointment at the orthopedic surgeon for treatment
- Client gives permission for the research and except the concequenses linked to the research
Exclusion criteria
- A operation less than two years ago in the shoulder area on the affected side
- A fracture less than 6 months old in the shoulder area on the affected side
- Suspicion of a fracture in the shoulder area on the affected side
- Malignancies or suspicion of malignancies in the shoulder area
- Neurological disorders that seriously affect shoulder function
- Limited understanding of the dutch language
