Outcome measures
Primary outcome
Primary outcome measures are functional limitations and pain. Functional
limitations are measured with the performance based 10 meter walking test and
the Timed up-and-go test and with the self report Patient Specific Complaints
questionnaire.
Secondary outcome
Secundary outcome measures is health related quality of life as measured with
the RAND-36 questionnaire.
Background summary
Chronic Complex Regional Pain Syndrome type 1 is a poorly understood chronic
pain disorder with regard to pathophysiology and treament, leading to severe
pain and disability. The efficacy of a wide array of treatment modalities is
disappointing.
An approach aimed at functional restoration seems to be promising. A
description and evaluation of such an approach has not been published yet.
Study objective
Aim of the study is to describe and evaluate an alternative physical therapy
management program in patients with a complex regional pain syndrome type 1. A
secondary aim is to describe the adverse effects that occur during or after the
intervention.
Study design
A descriptive double baseline pretest-posttest design is used. Treatment
duration depends on the clinical course of the disorder during the intervention
with a maximum of 16 interventions in 16 weeks. A follow-up is carried out six
months after the last intervention.
Intervention
The intervention under study is one of the treatment modalities currently
applied to patients with CRPS1 in Heliomare. Aim of the intervention is to
restore motility and mobility of the affected joints.
Restoration of joint mobility is accomplished by manual mobilisation and/or
manipulation. Manipulation is carried out by a high velocity-low thrust
technigue and is repeated with a maximum of 3 manipulations each treatment
session.
Restoration of joint motility is achieved by repetitive active movements partly
supported by the physiotherapist. Gain in both motility and mobility is
immediately applied to in activities of daily living, e.g. walking, running or
stair climbing.
And individually tailored exercise program is carried out in the days between
the treatment sessoins.
Increase in pain during or after the treatment sessions is ignored.
Study burden and risks
No adverse effects occured during or after treatment of 40 patients up till now
treated with this intervention. On average an increase in pain was measured
during the first 2 weeks of the intervention but pain intensity decreased in
the weeks thereafter dropping below the levels as measured at baseline.
Filling out two questionnaires, the SCL-90 and the Shortend MMPI-Dutch Version,
and an interview with a psychologist, is not routinely carried out during the
intervention and is therefore an extra burden for the participants. Two extra
measurement sessions, one at baseline and a follow-up measurement after 6
months are also not included in the routine treatment intervention.

Inclusion criteria
Eligible are patients with a chronic Complex Regional Painsyndrome type 1 according to the criteria of the IASP; pain for at least six months; age between 18 and 65; no other treatment for the last 4 weeks.
Exclusion criteria
Serious psychopathology based on the Symptom Checklist-90.
structural limitations of the joint mobility that affect walking without aids.
Patients willing to be treated according to the Dutch guidelines for the treatment of CRPS1.
Patients with limited skills in the dutch language.
