For the quantitative study the primary outcomes are:
a) the functioning of the nursing home resident who suffered a stroke:
physical, cognitive, emotional, social and communicative functioning, as well
the patient's capacity of decision making
b) the current care and treatment

a) the needs for care from the perspective of the resident
b) the needs for care from the perspective of the informal caregiver
c) the needs for care from the perspective of the professional

Background summary
40% of nursing home residents has stroke as the main cause of dependency.
Currently, the care offered to them focuses mainly on physical disabilities,
although many of them also suffer from cognitive, emotional and/or behavioural
changes, thay may result in unmet needs. In addition, they may also need
assistance with regard to the planning and decision making concerning their
care and (medical) treatment.
Study objective
The main objective of this study is to develop a care and treatment program for
stroke patients who are living in nursing homes, tailored to their needs.
The study consists of two parts. First, the functioning of stroke patients in
nursing homes will be explored in several domains (physical, cognitive,
emotional, social and communicative). This includes the evaluation of the
patient's capacity of decision making. Also current care and treatment will be
described, especially with regard to medical decision making and use of
restraints. The relation to patient characteristics (age, gender, education,
social network) and other determinants as stroke characteristics, comorbidity
and medication will be examined.
Second, the needs for care will be studied from the perspective of the patient,
the family and the professional. There will be a focus on the need for
information and the participation in medical decision-making. The results will
be compared to current care and treatment to discover 'unmet needs'.
Study design
The study is cross-sectional and explorative.
The first part is a quantitative study. The measurement instruments used will
be observation lists filled out by nursing home assistents and nursing home
physians.
The second part is a qualitative survey study. A selection of the patients and
their formal and informal caregivers will be interviewed on their (un)met needs.
Concept guidelines, based on the data analysis will be presented to a
multidisciplinary panel of experts.

Inclusion criteria
The patient:
- stroke is the main cause of his dependency and has occurred longer than 3 months ago.
- lives in a nursing home in a 'somatic unit' (a unit for residents who have mainly physical handicaps);The informal caregiver:
- is the person to be contacted or representative of the patient.
- is closely in touch with the care for the patient.;The professional:
- is responsible for the daily care of the patient
Exclusion criteria
The patient:
- has severe communication deficits
- can not be loaded due to severe depression or bad physical condition
- is not able to give informed consent
