Outcome measures
Primary outcome
Categorical data: sex, euroSCORE, ASA-score, comorbidity, educational status,
smoking status, number of grafts, number of complications.
Echographic data: TTE and TOE strain and strain rate.
Data will be described as mean and standard deviation.
Secondary outcome
Systolic myocardial function: Ejection fraction (2ch and 4ch), Fraction of
shortening, Fraction of area change, Stroke volume, dP/dt, Mitral tissue
doppler (systolic curve).
Diastolic myocardial function: Mitral wave (E/A ratio, E wave, A wave),
Pulmonary wave (S/D, S wave, D wave, A wave), Mitral tissue doppler (diastolic
curves).
NIRS: myocardial tissue oxygenation values.
Background summary
Myocardial function can be quantified with echocardiographic parameters of
Tissue Doppler and speckle tracking-derived strain and strain rate imaging.
Strain is a measure of myocardial deformation considered as a fractional change
in the length of the myocardium. It is expressed in a percentage with positive
or negative values, as consequence respectively of lengthening or shortening.
Validation studies have compared cardiac strain obtained with transthoracic
echocardiography (TTE) with tagged MRI and sonomicrometry. There are no studies
validating cardiac strain obtained with trans-oesophageal echocardiography
(TOE) as of yet.
We postulate also that there is a relation between echocardiographic parameters
of myocardial function and tissue oxygen saturation. Near infrared spectroscopy
(NIRS) is normally used for monitoring the adequacy of regional oxygen delivery
in the brain, kidney and gut. It `s never been evaluated if NIRS can be used
for the assessment of regional oxygen saturation of the myocardium.
Study objective
In this study we want to validate myocardial strain obtained with
transoesophageal echocardiography (TOE), compared with myocardial strain
obtained with transthoracic echocardiography (TTE), in patients undergoing
coronary artery bypass grafting.

Study burden and risks
The purpose of the study is to validate cardiac strain and strain rate measured
with TOE. Trans-oesophageal echocardiography is part of routine clinical
practice in cardiac surgery. In addition patients will undergo a transthoracic
echocardiography.
To measure myocardial oxygen saturation we will use NIRS electrodes that will
be applied on the thorax above the heart, but outside the surgical field.
This is a safe, non-invasive technique, with no known risks.

a.patients older than 18 years
b. patients who can give informed consent
c. ejection fraction (EF) > 50%
d. patients undergoing OPCAB or CABG
Exclusion criteria
a. patients who are unable to give consent
b. patients < 18 years
c. patients who are already part of another randomized study.
