Outcome measures
Primary outcome
Primary outcome will be patient recovery measured by the Dutch version of the Roland Morris Disability Questionnaire on several moments in time.
De primaire uitkomstmaat is de mate van herstel op verschillende tijdsmomenten, gemeten met de Nederlandse versie van de Roland Morris Disability Questionnaire.
Secondary outcome
Secondary outcomes will be the extent of carrying out daily activities, pain, patient’s use of health care, costs of health care.
Secondaire uitkomstmaten zijn verzuim van dagelijkse activiteiten en pijn, zorggebruik en zorgkosten voor deze episode van lage rugpijn bij huisarts en/of andere zorgverlener(fysiotherapie, specialist ect). Pijn (op de betreffende dag) zal worden gemeten met een VAS-schaal in het dagboekje en op 2,6 en 12 weken na het eerste consult.
Background summary

Acute and sub-acute aspecific low back pain is an common ailment in general practice. Dutch guidelines offer multiple options for treatment, a single preferred treatment is not described. There is a clinical equipoise and contextual factors, such as the patient’s illness and treatment expectations seem to have a large role in the recovery of patients with low back pain. We expect that patients will recover faster when GPs ask about patients’ illness and treatment expectations and take these expectations as a starting point for the process of reaching a shared decision on the therapeutic plan.
In this clustered randomized controlled trial 60 GPs will see 600 patients with non-chronic low back pain. Thirty GPs will provide care as usual, while the other 30 GPs will be trained to perform their consultation using shared decision making. These GPs will specifically ask about the patient’s expectations and will decide on the treatment plan together with the patient. The effect of this intervention on patients’ speed of recovery, pain, use of health care and costs of health care will be measured.
Study objective
We expect that patients will recover faster when GPs ask about patients’ illness and treatment expectations and take these expectations as a starting point for the process of reaching a shared decision on the therapeutic plan.
De verwachting is dat het herstel van patiënten spoediger zal verlopen als :
1. Artsen de verwachtingen van de patiënt uitvragen;
2. Op basis van deze verwachtingen expliciet overeenstemming wordt bereikt over het te volgen therapeutisch plan.
Study design
2, 6, 12 and 26 weeks after the first consultation, 6 months for use of health care only.
2, 6, 12 en 26 weken na het eerste consult en na zes maanden voor zorggebruik.
Intervention
The GPs in the intervention-group will ask patients about their expectations concerning recovery and treatment. GPs will take these expectations into account when deciding on a therapeutic plan. The therapeutic plan will be decided on in a process of shared decision-making. GPs will take part in two training sessions of 2.5 hours to learn to apply the intervention.
De artsen in de interventiegroep ontvangen tweemaal een training van 2 uur en 30 minuten(36) over het vragen naar verwachtingen van de patiënt en de wijze van SDM met gebruikmaking van een keuzehulp.

Inclusion criteria
Adults(between 18-65 years), who visit their GP because of a new episode of non-chronic low back pain as described in the guideline low back pain of the Dutch General Practinionars Association(M54).
Volwassen personen (18-65 jaar), die de huisarts bezoeken vanwege een nieuwe episode van niet-chronische aspecifieke lage rugpijn, zoals verwoord in de standaard lage rugklachten van het NHG (M54).
Exclusion criteria
1. Onset of the complaint was more than 3 weeks at the time of consultation;
2. The back pain is a recurrence within 3 months after the onset of earlier back pain;
3. Pregnancy;
4. Insufficient understanding of the Dutch language, as judged by the GP.

2. Een recidiefklacht binnen 3 maanden na de oorspronkelijke klacht van niet- chronische lage rugpijn;
3. Zwangerschap;
4. Onvoldoende begrip van de nederlandse taal, beoordeeld door de huisarts.
