Brief title
Health condition
Primary objectives are to describe the frequency and type of workarounds in the BCMA process and to determine the association of workarounds with medication (administration) errors in the BCMA process. Secondary objective is to explore the potential risk factors (determinants) for workarounds.

Outcome measures
Primary outcome
The primary endpoints of the study are the number and type of workarounds, and the number, type and class of medication errors.
Secondary outcome
As secondary endpoints determinants for workarounds will be collected
Background summary
New technology based methods such as bar code assisted medication administration systems (BCMA) should reduce administration errors in hospitalized patients but user interpretation of new procedures by staff (nurses and doctors) can lead to workarounds and as a consequence to medication administration errors and patient harm.
