Primary outcome
Microcirculatory changes induced by xenon or sevoflurane anesthesia.

Background summary
This study concerns the assessment of microcirculatory alterations induced by xenon or sevoflurane anesthesia in patients with compromised left ventricular systolic function. We will measure the microcirculation with a sublingual echo and we will combine this with pulse contour method to follow the cardiac output and heart function.
Study objective
The aim of the study is the assessment of microcirculatory alterations induced by xenon or sevoflurane anesthesia in patients with compromised left ventricular systolic function.
Study design
Once the adequate anesthetic level (i.e. .6 of Xenon or 1 MAC sevoflurane concentration measured by the anesthetic machine) is obtained microcirculation is assessed.
Intervention
Microcirculatory alterations and hemodynamics will be assessed before and after start of xenon or sevoflurane anesthesia.

Inclusion criteria
- Patients with left ventricular ejection fraction <30%
- Undergoing the insertion of a defibrillator
- Age >18y-75y
Exclusion criteria
- Patients with ASA I and ASA II
- Age <18y
- Normal LV function
