Outcome measures
Primary outcome
Effect evaluation:
- Length of hospital stay;
- Appropriate antibiotic treatment according to the generic quality indicators
Secondary outcome
Effect evaluation:
- admission to and duration of intensive care unit stay;
- readmission within 30 days;
- mortality.
Process evaluation:
- doctors’ compliance with the checklist
Economic evaluation:
- total antibiotic use;
- costs associated with resource use and hospital stay;
-costs related to implementation and use of the checklist.
Background summary
The aim is to develop and provide an antibiotic checklist based on validated generic quality indicators measuring the appropriateness of antibiotic use in hospitalized adults treated for a suspected bacterial infection, and to evaluate the introduction of the checklist in terms of (cost)- effectiveness. We will introduce the checklist in 9 Dutch hospitals, using a stepped wedge design.
Study objective
Applying to the antibiotic checklist - based on generic quality indicators for the antibiotic treatment of bacterial infections in the hospital - will likely result into better patient care in terms of adequate treatment, duration of intensive care unit (ICU)- and hospital stay, mortality, the total antibiotic use and costs.

Intervention
An antibiotic checklist based on generic quality indicators defining appropriate antibiotic use.
The antibiotic checklist is meant as a reminding tool for the doctor who starts intravenous (IV) antibiotic treatment. It is divided in two parts; the first part has to be completed at the moment of prescribing IV antibiotics, and consists of five items. The second part of the checklist is applicable during the course of treatment, at the latest after 72 hours of treatment, and consists of two items.

Inclusion criteria
-age ≥ 18 years old
-hospitalized patients or patients who will be hospitalized and present at the ED
- a suspected community-acquired and/or- hospital-acquired bacterial infection
- treatment with antibiotic therapy that starts intravenously.
Exclusion criteria
- anticipated hospital stay of less than 24 hours;
- antibiotics used as prophylaxis or < 24 hours antibiotics;
- hospitalization at the ICU during start of the treatment;
- transfer from another hospital;
- antibiotic treatment in the ambulance, before presentation at the ED.
