Primary outcome
Percentage of patients with underlying malignancy who died outside the hospital.
Secondary outcome
1) average number of days in hospital in the last 3 months of life

A. Percentage of patients of whom the start of the palliative phase has been marked ('bad-news'-interview)
B. Percentage of patients of whom treatment restrictions has been reported
C. Percentage of patients preference 'where to die' has been reported
3) Percentage contact physician/nursing home physician in the last phase of life and after death.

A. Percentage of patients who underwent a systemic anti-tumor treatment (chemotherapy) in the last 3 months of life
B. Percentage of patients with interventions (diagnostic / treatment) in the last 3 months of life
5) Quality of life and quality of care in the palliative terminal phase of the patient from the point of view of the survivors
6) Percentage patients where a pain-team / spiritual counselor / psychologist /social worker was involved in the last 3 months of life
Background summary
Dit studieproject betreft een interventie-onderzoek met een voor- en een nameting. De interventie bestaat uit het implementeren van het Zorgpad Palliatieve Zorg bij patiënten met kanker op de afdelingen Oncologie en Longziekten .
Het doel van dit onderzoek is na te gaan of het gebruik van een Zorgpad Palliatieve Zorg in het Maasstad Ziekenhuis de kwaliteit van de palliatieve zorg voor patiënten met kanker verbetert. Het onderzoek heeft als primaire vraagstelling: leidt het gebruik van het ZPZ tot een stijging van het percentage patiënten met kanker dat buiten het ziekenhuis overlijdt. Als secundaire vraagstellingen zal worden onderzocht of het gebruik van het Zorgpad leidt tot minder ziekenhuis-opnamedagen in de laatste drie maanden van het leven, betere verslaglegging in het medisch dossier, betere communicatie naar de eerste lijn, minder onderzoeken / behandelingen in de laatste drie maanden en een betere kwaliteit van leven van de patiënten, bezien vanuit het oogpunt van de naasten.
Het onderzoek zal plaatsvinden op de afdelingen Oncologie en Longziekten in het Maasstad Ziekenhuis. Via het programma Trits worden patiënten geïdentificeerd die overleden zijn nadat ze eerder voor hun oncologische aandoening door deze afdelingen behandeld zijn. Data verzameling vindt plaats vanuit dossieronderzoek en middels een vragenlijst naar kwaliteit van leven ingevuld door nabestaanden. De voor- en nameting zullen beiden naar schatting één jaar in beslag gaan nemen.

Implementation of the 'Pathway Palliative Care' may lead to a rise in the percentage of patients with cancer who die outside the hospital.

Inclusion criteria
The departments of Oncology/Hematology and Lung diseases of the Maasstad Hospital participate in the study. Patients over 18 years of age, who died of an oncological disease and that were known at the outpatient clinic and/or clinic of the relevant sections are included.
Exclusion criteria
The patients who were referred to another hospital for further treatment and/or guidance of the disease are excluded in the study.
