Background summary
The Carpal tunnel syndrome is a frequent occuring compression neuropathy of the upper limb. Literature shows that surgery of the wrist is the most effective intervention for the treatment of Carpal tunnel syndrome. However surgery often leads to complications like instability and rather occuring degeneration of the wrist. Furthermore non-invasive interventions like braces, neurodynamic mobilization, Low Level Lasertherapy and ultrasound have positive effects in treating Carpal tunnel syndrome. But these are mostly short term effects. Because of these insufficient results of the different interventions more research is necessary to find an alternative non-invasive intervention for the treatment of Carpal tunnel syndrome. The aim of this study is to investigate the influence of focused extracorporeal shockwave therapy in the treatment of patients with Carpal tunnel syndrome.
Study objective
Focused extracorporeal shochwave therapy shows positive effect in the treatment of tendon disorders for example of the shoulder, elbow or by plantar fasciitis.
Study design
Five treatments during a five-day interval with a follow up four weeks after the last intervention.
Intervention
Focused extracorporeal shockwavetherapy.
Inclusion criteria
1. Night pain and paresthesia of the hand
2. Functional impairments in daily life
3. Positive Phalen sign
4. Positive Tinel sign
5. Age older than 18 years
6. Goed mental cognition en sufficient knowledge of the Dutch language
Exclusion criteria
1. Treatment with corticosteroid- and hyaluroninjections
2. Invasive of non-invasive treatments in the past, for example brace, neurodynamic mobilization, Low Level Lasertherapy and ultrasound
3. Pregnancy
4. Carpal tunnel syndrome symptoms caused by polyneuropathy
5. Cervical radiculopathies
6. Neck related disorders, for example Thoracic Outlet Syndrome or peripheral nerve entrapment
7. Hand related traumas (luxation of the carpalia or bonefracture)
8. Surgery of the hand/ wrist with placing of osteosynthesis material
9. Open wounds of the hand/ wrist
10. Mental instability
