Outcome measures
Primary outcome
The primary outcome measure is the difference in the ground reaction force of the affected leg relative to the unaffected leg measured 4 weeks postoperatively while performing a gait analysis on the GRAIL. The symmetry of this outcome measure is compared between the DSA and the PLA group.
Secondary outcome
The gait analysis on the GRAIL also provides information about parameters such as; ROM, EMG Data, PROMs etc. These will also be compared between the DSA and PLA groups.
Background summary
Rationale: The Total Hip Arthroplasty (THA) is one of the most successful orthopedic operations. The aim of the operation is to remove the pain and functional limitation associated with coxarthrosis, so that patients can function independently again. However, the clinical results of the minimally invasive techniques on these outcome measures vary. For example, different studies with different minimally invasive approaches show hardly any significant differences with regard to gait in the later recovery phases. The earlier significant differences that were found, are demonstrated in the early phase after the THA.
In recent years, several new minimally invasive techniques have been developed to perform the THA. The purpose of these minimally invasive approaches is to limit muscle damage during THA, which would reduce postoperative pain and have a faster postoperative recovery.
One of the newest minimally invasive techniques is the Direct Superior Approach (DSA). This is a tissue-saving approach whereby the iliotibial tract is left intact compared to the standard Postero-Lateral Approach (PLA). In theory, this approach should reduce postoperative pain and a faster functional recovery. However, this has not been investigated yet.
Study objective

In theory, this approach should reduce postoperative pain and has a faster functional recovery.
Study design
Measurements on the GRAIL will take place after 4, 8 and 12 weeks after the operation
Inclusion criteria
- On the waiting list for a total hip arthroplasty because of artritis
- having a BMI<35
Exclusion criteria
- having a BMI>35 hebben
- an earlier joint replacing operation of the lower extremities in the last year
