Background summary
A lot of research has been done on work-related psychological problems and
coping. Less is known about the psychopathology of these patients relative to
individuals with psychiatric problems as depression or anxiety disorders.
Changes in biological stress markers (salivary cortisol profiles) have been
described in psychiatric patients, but much less in individuals with
work-related psychological problems.
Study objective
The main objective of this study is:
To compare patients referred to a specialized clinic for work-related problems
(SKILS) with patients known with common mental disorders in primary and
secondary care and healthy controls with respect ot dimensions of
psychopathology.
Additional objectives are:
To compare patients referred to a specialized clinic for work-related problems
(SKILS) with patients known with common mental disorders in primary and
secondary care with respect to DSM-IV diagnoses, diurnal saliva cortisol
profiles and post-dexamethason cortisol levels; Healthy subjects will also be
included in the comparison of cortisol-levels.
To investigate the relationship between the stress syndromes according to the
definitions used in occupational health medicine and the DSM-IV diagnoses.
To investigate the relationship between the effect of treatment and changes in
diurnal cortisol levels in subjects undergoing routine treatmenst for
work-related problems in SKILS.
To investigate the reliability and validity of the Dutch translation of the
CD-RISC, an instrument to measure resilience in general.
Study design
Observational study, cohort study.
Study burden and risks
Participants are on sick leave or are at risk to be in the near future if
nothing changes. The occupational health doco\tors send them to SKILS becasus
an assessment and intervention is necessary to help the people with
rehabilitation. In that sense there is no burden to participate in this study.
The extra burden lays in the sampling of saliva two times, two times the intake
of one tablet of Dexmethasone and two times an extra questionnaire, the
CD-RISC.

Between 18 and 65 years of age
Employed
On sick leave or threatened to go on sick leave in the near future
DSM IV Diagnosis: adjustment disorder (distress, surmenage, burnout)
Exclusion criteria
Not able to speak or read Dutch.
