Background summary
Vertebral fractures occur frequently among geriatric patients. Usually the
underlying diagnosis is osteoporosis. It is shown that having a fracture is
associated with an increased risk of subsequent fractures. Therefore effective
osteoporosis drugs should be administrated. A lot of vertebral fractures are
overlooked for several reasons. A recent retrospective study in our center
showed a prevalence of 50% of vertebral fractures on the X-ray of the lateral
Thorax, among geriatric patients presenting on the dayclinic. 50% of these
fractures were diagnosed bij the radiologist, and only 7% was treated with a
bisphosphonate, extra Calcium and Vitamin D.
Problably this prevalence is underestimated because most vertebral fractures
are in de lumbosacralspine and not visible on a lateral Xray of the thorax.
Because of these results, the standard analysis of the new patients on the
geriatric dayclinic has a thoracic and lumbosacral spine Xray.
Study objective
Prevalence of vertebral fractures on the thoracic lumbosacralspine X-ray.
Study design
prospective study on patients presenting on the geriatric dayclinic in the
Slotervaart Hospital Amsterdam. 300 patients will be included. The usual
analysis for this patients will be performed and will have a X-ray of the
toracic lumbosacral spine.
Study burden and risks
The thoracic and lumbosacralspine Xray is not an extra burden, because this
Xray will be made not for scientific research but for the sake of the patients.
We conduct this thoracic and lumbosacralspine Xray in the standard analysis for
all new geriatric patients on our dayclinic.
In 50% of the patients the minimal mental state examination (MMSE) will be
performed extra, normally we take this test from the ones with memory
complaints. The burden wil be 10 minutes, and there is no risk.

patients presenting for geriatric analysis in slotervaart hospital Amsterdam, and from who are made an X-ray of the thorax and of the thoracic and lumbosacral spine.
Exclusion criteria
patients without an X-ray of de thoracic and lumbosacral spine.
