Primary outcome
Effects will be assessed in terms of changes in the prevalence of DRP.
Secondary outcome
Secondary endpoints are the incidence of re-hospitalisation, attitude to drugs,
persistence of drug adherence, quality of life and cost-effectiveness.
Background summary
Drug related problems (DRP) such as contra-indications, interactions, adverse
drug reactions (ADR), incorrect dosing, irrationality and inefficacy of
treatment not only result from the specific effects of drugs in patients but
also from causes like prescription errors and non-compliance with treatment.
Risk factors for DRP include aging, co-morbidity, polypharmacy and
hospitalisation.
Study objective
The main objective of this study is to examine the effect of medication review
and counselling of discharged patients, using five or more prescription only
drugs, by the community pharmacist on the prevalence of Drug Related Problems
(DRP).
The effect of patient counselling by pharmacists on re-hospitalisation,
compliance, quality of life and the cost-effectiveness of the intervention are
secondary objectives.
Study design
A randomised controlled trial.
Intervention
A systematic medication review by community pharmacists. Pharmacy technicians
will counsel patients at home at baseline and at 1, 3, 6, 9 and 12 months.
Control patients receive usual care.
Study burden and risks
Patients are asked to fill in a questionnaire containing questions about
patients* health state and compliance. Patients are furthermore asked to fill
in questions about their health care consumption to measure cost effectiveness.
At baseline patients are counselled by pharmacists during a homevisit about the
effects, side effects and use of the drugs, and compliance. Over a period of
one year patients get additional counselling at 1, 3, 6, 9 and 12 months.
Participants may benefit of changes in behavior, which reduces the risk on the
occurrence of drug related problems and re-hospitalization. There are no risks
associated with trial participation.

Inclusion criteria
Age: over 60 years
Drugs: over 5 chronic prescription only drugs
Discharged from hospital from the departments of cardiovascular disease, lung disease and internal medicine
Exclusion criteria
- Patients discharged to a nursing home
- A poor mental status
- Terminal ill patients
- Patients not able to communicate in Dutch
