Primary outcome
Days to completing the last phase of a prefabriced running scedule from the
moment of randomization.

Background summary
One of the most common causes of exercise related leg pain is medial tibial
stress syndrome (MTSS). The literature suggests an incidence between 4 and 35%
in mainly military populations. Lost of controversie exists for the name for
the condition; MTSS has had many names in the past, such as shin splints, which
is commonly used.
Clinically patients with MTSS suffer from pain on the posteromedial side of the
tibia during exercise and om physical examination pain is present for at least
five centimeters on the posteromedial border of the tibia. The differential
diagnosis compartment syndrome and stress fracture is excluded clinically.
Very limited studies were conducted for treatment of MTSS. One study was
conducted in 1974, one in the nineties and one recently in 2006. None of these
studies showed evidence for treatment of MTSS than rest alone. In practise,
usually prescribed treatments are: strengthning and stretching of the calf
muscles, orthotics, anti-inflammaroty medication and massage are prescribed,
but none of these are scientifically based treatments.
Herzog Medical developped a compression sleeve for the calf for a multitude of
calf complaints. A pilot studie was conducted for wearing comfort and was
graded sufficient. Compression on bone leads to stimulation of thye osteoblasts
and osteocytes in the tibial cortex through increased flow in the canaliculi.
This could help patients with MTSS.
Study objective
Decrease the number of days complaints from MTSS through wearing of a
compression sleeve or physical therapy exercises (strengthning and stretching
of the calf muscle with additionally balance exercises of the ankle).
Study design
A three-armed randomized trial; a sportscompression sleeve, physical therapy
with or without addition of standard treatment in the form of a rehabilitation
running program.
Intervention
Wearing of a sports compression sleeve around the calf or a physical therapy
program consisting of strengthning and stretching exercises.
Study burden and risks
Limited, the pilot study showed that complaints of wearing of the sleeve were
minimal and the physical therapy program is not very heavy work.

Inclusion criteria
Age over 16. Pain on the posteromedial border of the tibia during exercise and on palpation for at least five centimeters.
Exclusion criteria
Clinical suspicion on stress fracture or compartment syndrome, operation in the involved area.
