Background summary
Most people in the Netherlands do not look forward to live in a nursing home,
because of the (supposedly) undignified conditions people live in. Loss of
dignity has consistently been found as one of the most frequently mentioned
reasons for requesting euthanasia or physician-assisted suicide and
experiencing a loss of will to live. However, there is lack of insight in what
constitutes dignity. Chochinov et al. developed an empirical model of dignity
in terminally ill cancer patients. It is necessary to further test and refine
the model for other populations, e.g. nursing home patients. An important topic
to focus on, is the influence of care and care setting on dignity.
Study objective
To further develop the dignity model in the nursing home setting by assessing
which factors are especially relevant to dignity in this setting. The specific
objective of this substudy is to develop a valid and reliable measurement
instrument to measure factors that influence the personal dignity of nursing
home patients.
Study design
A cross-sectonal study in which 100 nursing home patients admitted to somatic
wards of nursing homes will receive a questionnaire, which asks whether a
certain aspect (e.g. level of dependency, mental health, privacy, being treated
with respect, autonomy) is present, and, if yes, to what extent this influences
their personal dignity. Furthermore, per nursing home patient a relative, an
elderly care physician and a nurse will receive the questionnaire.
Study burden and risks
There are no risks associated with participation. However, the patient may
become emotional, because patients are confronted with their illness and
personal dignity. The main burden for the physicians, nurses and relatives will
be an investment of time. Future nursing home patients, their relatives and
caregivers may benefit from this study.
