Background summary
Case management is a way to ensure that care is delivered according to
patients' wishes. In several regions in the Netherlands case management is
implemented. This is not always done under the name of casemanagement and also
implementation stategies vary.
In a national count in 2010, 20 initiatives for case management were identified
in the Netherlands. There is little research into these initiatives.
Research of case management in palliative care comes mainly from the United
States. Results vary but indicate some positive effects (e.g. in advace care
planning). Research in the Dutch setting is necesary to gain insight in
usefullness of case management in the Dutch care system.
Study objective
To gain insight in mechanisms of case management and in succesful and less
succesful aspects of case management in individual patient care. This leads to
knowledge of potential added value of case management to regular care and (if
there is added value) a recomendation on implementation of case management.
Study design
A retrospective study involving informal carers of deceased patients who have
received case management.
The general practitioner, district nurse and case manager will also be
interviewed, after the informal carer has given permission.
Study burden and risks
Informal carers may experience emotional distress. Future patients and carers
receiving palliative care and case management in palliative care may benefit
from this study.

Inclusion criteria
Informal carers of deceased oncology patients, their GP, district nurse and case manager
Informal carers of deceased oncology patients who have received case management (contrast group: informal carers of patients who have received palliative care from their GP)
Able to understand their participation and able to give informed consent
Exclusion criteria
Patients who's death was unexpected and/of sudden
Patients who received curative care at time of death
Informal carers unable to communicate in the Dutch language
