Primary outcome
The primary outcome measure is: reduction in the frequency and intensity of
suicidal thoughts.
Secondary outcome
The secondary outcome measures are: self-harm, suicide attempt, suicide
ideation attributes, depression symptoms, hopelessness, anxiety, quality of
life worrying and satisfaction with the treatment.
Background summary
The Turkish speaking migrant population is one of the high risk groups for
suicidal behaviours compared to the other ethnic communities in the Netherlands
as well as in other countries in Europe (e.g. Germany, the United Kingdom). The
threshold for seeking professional help is high, and the use of the mainstream
health services within this community is low. It is estimated that 80 % of the
Turkish community, living in the Netherlands, have access to internet. The
internet is potentially an innovative way to reach this population. The
effectiveness of a CBT online self-help intervention on suicidal ideation for
the general Dutch population was demonstrated in a previous RCT. In the current
study the intervention will be adapted and the effectiveness will be tested for
Turkish migrants.
Study objective
The present research aims to evaluate the effectiveness of a culturally
adapted, guided and CBT-based online intervention for Turkish speaking
migrants presenting suicidal ideation.
Study design
This study is a RCT with 2 conditions: experimental and waiting list control.
The experimental group obtains direct access to the guided web-based self-help
intervention which takes 6 weeks to complete. Participants in the waiting list
condition obtain access to the modules after 6 weeks. Participants in both
conditions are assessed at baseline, post-test (6 weeks) and followed up 3
months after post-test.
Intervention
The self-help intervention consists of 6 weekly lessons. Every lesson contains
information, example patients and exercises. By completing these exercises,
participants learn to gain control over and reduce their suicidal thoughts.
Attention is also given to self-harm, emotion regulation, future perspectives
and self-esteem. Trained bi-lingual moderators will be available to provide
guidance in working through the lessons.
Study burden and risks
Safety protocol will be used which was used previously by Van Spijker (2012).
This means that suicidal ideation is bi-weekly checked. The participant and his
/ her general practitioner will be contacted and referrals to other
institutions (e.g. Maytree in the UK and 113 online in the Netherlands) will be
made in case of elevated risk.

Inclusion criteria
Turkish immigrants in Holland aged 18 years and above with mild to severe suicidal ideation. In the current study, persons scoring 1 and above on the Beck Scale for Suicide Ideation (BSS) are eligible to be participants.
Exclusion criteria
Persons under the age of 18 years or insuffient knowledge of the Turkish or Dutch language.
