Primary outcome
The primary parameters of interest are the YSQ and SMI, in order to validate
these instruments. The YSQ and SMI are self-report questionnaires. Within a
subgroup of 60 patients, the YSQ and SMI will be re-administered within one
week to examine the test-retest reliability. For the second question, schema
activation (YSQ scores) among nursing home residents will be examined by
comparing cases and controls, stratified for the presence of a psychiatric
disorder using multivariate analysis of variance adjusted for potential
confounders/group differences.
Secondary outcome
To examine the convergent and divergent validity, sociodemographic
characteristics, personality functioning, early traumatization, (instrumental)
activities of daily living, alcohol use, psychological distress, mental
wellbeing, and cognitive functioning will be assessed with well-validated
questionnaires for older people. Used are self-report questionnaires, and two
brief neuropsychological screeners. All are administered by the researcher.
Background summary
The health care system in the Western world, especially the Netherlands, is
organized to maximally support independent living as long as possible.
Inevitably, patients admitted to a nursing home are becoming much more complex,
often having mental-physical multimorbidity.
Little is known about the effect of personality functioning on the significant
challenge that this group faces, namely adaptation to institutionalization.
Study objective
This study is set up 1) to validate the Young Schema Questionnaire (YSQ) and
Schema Mode Inventory (SMI) aimed to explore early maladaptive schemas and
schema modes underlying personality functioning, and 2) to explore schema
activation related to the nursing home setting which is a first step in the
development of schema-focused therapy in the latest stages of life/a nursing
home population.
We hypothesize that the YSQ and SMI are valid and reliable indicators of
personality functioning in older people and that, irrespective of the presence
of a psychiatric disorder, specific maladaptive schemas are activated in
nursing home residents.
Study design
A cross sectional study design.Two parallel case-control studies, i.e. one
comparing patients admitted to a somatic ward of a nursing home (cases, n=75)
with geriatric outpatients (controls, n=75) and one comparing patients admitted
to gerontopsychiatric ward of a nursing home (cases, n=75) and psychiatric
outpatients with a personality disorder (controls, n=75).
Study burden and risks
Non-dementia nursing home residents can be vulnerable due to somatich,
cognitive and psychiatric factors. In this study we sought to minimize the
burden of the one time assessment. Where available, short forms of
questionnaires were selected. Assessments are conducted by the researcher and
specifically trained assistents. A familiair environment is selected for the
administratien: the participants room or a designated room in the nursing home
unit. Participants are verbally assisted if they are not able to fill in the
questionnaires. Testload will be actively monitored by the assesor and if
desirable, the assessment will be split up in smaller portions. A pilot
assessment was conducted by a gerontopsychiatric nursing home resident with
profound deficits in attention, resulting in a total duration of two hours,
distributed over four sessions of 30 minutes.

Inclusion criteria
- Age 50 years or older.
- Resident of a gerontopsychiatric (cases group 1) or somatic (cases group 2)
nursing home ward (for cases) or referral to an outpatient clinic for old age
psychiatry (controls group 1) or an outpatient geriatric clinic (controls
group 2).
- Giving informed consent after oral and written study information.
Exclusion criteria
- Insufficient command of the Dutch language.
- A current severe mental illness such as active bipolar disorder, psychotic
disorder or active substance use disorders.
- Full legal incapacity.
- An established (or suspected) diagnosis of dementia.
- Physically or mentally not able to fill out the necessary questionnaires.
- A history of or current psychiatric treatment within specialized mental
health care (only for cases group 2 and controls group 2).
- A major neurocognitive disorder, i.e. delirium, dementias and extensive
acquired brain damage accompanied by severe communicative problems and/or
impairments of the sense of self thus impeding the ability to self-report.
