Background summary
An increasing amount of people in the Netherlands are requesting euthanasia due
to severe psychological suffering. Most people in this group struggle with a
death wish for years, have a high risk of dying by suicide, and lack access to
timely and appropriate care. To prevent these people from dying lonely and
distraught by suicide, more knowledge is needed about the nature of death
wishes, death thoughts and suicidality, as well as the care needs within this
group.
Study objective
1. To gain insights into care needs with regard to death wishes, death thoughts
and suicidality of people requesting euthanasia due to psychological suffering;
2. To gain insights into characteristics of death wishes, death thoughts and
suicidality among people making euthanasia requests due to psychological
suffering;
3. To gain insights into the course of these characteristics over time, i.e.
after placement on the waiting list, after rejection and/or after withdrawal of
the euthanasia request;
4. To draw up a concrete plan for the development of a care program aimed at
providing guidance to those seeking help and at preventing suicide among
applicants for euthanasia due to psychological suffering.
Study design
Prospective longitudinal observational study. Participants will receive a set
of questionnaires twice: a first measurement after registration at EE is
complete, and a repeat measurement 180 days after the first measurement. By the
time of the second measurement, 90% of all participants are expected to have
completed triage, after which they a) are on the waiting list for an
exploratory interview or for comprehensive examination ("waiting list"), b)
have been rejected by the triagist ("rejected"), or c) have decided not to
pursue the request ("withdrawn").
Study burden and risks
Research activities consist of asking participants twice about their death
wishes, death thoughts and suicidality through a questionnaire. The
questionnaires can be perceived as burdensome by participants. Multiple studies
have shown that questioning people about suicidality has no harmful effects,
and may even have positive effects.
