Effects on hotspotters’ acute medical care use in society.
Achtergrond van het onderzoek
There is a large gap between the rich and poor in the Netherlands. In 2010 the life expectancy gap between these two groups was 7 years. This finding has been linked to the socioeconomic status (SES).
With data collected from a previous study, we were able to identify a low SES group, living in a deprived living area with complex health problems and deviant use of healthcare. Gawande et al defines this group as hotspotters. The deviant use of healthcare that has been observed among hotspotters is also known as ‘mismatch of care’. Mismatch can be defined as over-, under- or misfit care with negative effect on the quality and quantity of the patient’s life.
Currently general practitioners do not have guidelines for this complex group, even though the healthcare professional and patient are influenced heavily by this problem.
At this moment in time each patient undergoes the same healthcare process (care as usual) at the general practitioner: diagnostics, management protocol and evaluation of the protocol. When evaluating hotspotters, the question about whether or not a different or additional approach should be used to help these patients arises.
To be able to find the best suited guidelines, there is still more insight needed about the healthcare process of hotspotters at the general practitioner.
The research question of the current study is: what is the effect of the positive health tool on the acute medical care use and participation of people with complex problems in various life domains.
We shall be evaluating the registration, follow-up policy of the general practitioner and the effects of this method for the healthcare professional and patient.

0, 3, 6 and 12 months.
Onderzoeksproduct en/of interventie
The participating patients will be invited to the general practitioner for a diagnostic interview with an onsite mental health specialist. The patients will then be interviewed by either the mental health specialist or the general practitioner with the positive health tool.

Belangrijkste voorwaarden om deel te mogen nemen (Inclusiecriteria)
The patients are:
- 18 years or older.
- competent to make decisions concerning their health
- Have ≥2 out of the 3 following problems: psychiatric, chronic disease or social problem.
Have more than average acute medical care usage ( ≥2 times per year).
Belangrijkste redenen om niet deel te kunnen nemen (Exclusiecriteria)
The exclusion criteria done by the general practitioner:
• The patient is terminal.
• The patient has dementia and is unable to communicate effectively.
• the patient is lacking decision making capacity (concerning their health).
• The patient doesn’t give permission to participate in the study.
